Speech-Language Pathologist, Home and Community Care – Temporary Part-Time (0.7) (Until March 2027)

Position Summary

The Speech-Language Pathologist (SLP) provides speech language therapy services to CHEO patients in homes, schools and other areas as needed, through conducting clinical assessments, and developing treatment plans and goals, in consultation with the patient, family and other interdisciplinary team members, in accordance with the standards of the College of Audiologists and Speech-Language Pathologists of Ontario (CASLPO). The Speech-Language Pathologist provides recommendations for follow-up and management of the patient's condition. The Speech-Language Pathologist will support the School-Based Rehabilitation Service (SBRS), providing assessment and/or treatment for children 4 to 21 years of age. The Speech-Language Pathologist will support CHEO's School-Based Rehabilitation Services (SBRS) Program by providing care in a variety of settings including in schools, CHEO sites, various community sites, virtual and other settings, as needed. The Speech-Language Pathologist will support the SBRS program across the entire Champlain region of Eastern Ontario and must be willing and able to travel long distances independently on a daily basis as needed.

Main Responsibilities

• Completes assessments of patients in a variety of treatment settings including private homes and schools, CHEO sites, community sites and others and performs an analysis based on objective, subjective and functional findings.
• Establishes realistic, measurable and time limited treatment goals and develops treatment plan.
• Ensures that speech-language therapy treatments are initiated with the informed consent of the patient or substitute decision maker in accordance with the Health Care Consent Act.
• Provides direct treatment and/or support and teaching to informal caregivers, school personnel and other health care professionals involved.
• Participates in case conferences when required and consults with other members of the health care team in fostering a holistic approach to care.
• Completes reassessments to review patient's progress and treatment goals and reports to Care Coordinator and other members of the health care team in order to ensure continuity of care and determine if other resources and referrals are required.
• Plans in collaboration with the patient, family and Care Coordinator, for the termination of service when patient's goals are achieved and/or when patient no longer requires the service.
• Completes documentation and maintains appropriate patient records in accordance with the standards of CASLPO and CHEO policies and procedures.
• Responsible for teaching patient, family and other team members, the treatment program and management.
• Acts as a resource for CHEO employees, service providers, school personnel, and community by providing educational programs as requested.
• Supports caseloads associated with Communicative Disorders Assistants (CDA) as well as student placements.
• Attends and participates actively in team meetings and general staff meetings.
• Assumes responsibility for maintaining professional knowledge and updated skills.
• Prioritizes Speech Language Therapy services in accordance with the CHEO policy and the School-Based Rehabilitation Service program mandate and requirements.
• Follow organizational policies and procedures related to safety, including reporting and follow-up for incidents/accidents and near misses, identification and management of risks and emergency procedures.
• Perform other related duties as assigned by Manager.

Qualifications

• Master's Degree in Speech-Language Pathology (Essential)
• Minimum two (2) years' experience as a Speech-Language Pathologist, preferably in pediatrics (Essential)
• Related experience in a community-based or school-based setting (Essential)
• Experience working with and supporting children and youth in developmental education settings with complex health profiles (Essential)
• Registration with the College of Audiologists and Speech-Language Pathologists of Ontario (CASLPO) (Essential)
• Current CPR-C certificate (Preferred)
• Communicates effectively, orally and in writing (Essential)
• Demonstrates excellent interpersonal skills (Essential)
• Excellent time management and organizational skills (Essential)
• Comfort level and skill in preparing and facilitating therapeutic groups (Essential)
• Flexible hours including evening, weekend, and holiday work may be required (Essential)
• Ability to travel to provide service to clients throughout all of Ottawa and the Champlain region (Essential)
• Ability to attend last-minute meetings in-person, as needed (Essential)
• Valid driver's license, access to a vehicle, and the ability and willingness to travel from site to site and to complete multiple community-based visits on a daily basis is required and as assigned. (Essential)

Effective March 29, 2021, CHEO will conduct a more comprehensive pre-screening and evaluation process.
